Nurse Practitioner - Nephrology

Job Details

• Duration: 26 weeks
• Hours per Week: 40
• Shift: 8 Hour Days, Monday to Friday
• Employment Type: Local, On-Site

Job Requirements

• Minimum of two (2) years' experience as a Nurse Practitioner (NP)
• Current unrestricted NP license to practice in the state
• Current Drug Enforcement Administration (DEA) license required or eligible to obtain within 90 days of hire
• Current Cardiopulmonary Resuscitation (CPR) or Basic Life Support (BLS) certification
• Intermediate computer skills and proficiency in MS Word, Excel & Outlook required
• Preferred experience in Value Based Care or Population Health
• 5+ years’ experience as a Family Nurse Practitioner (FNP), Primary Care Nurse Practitioner (NP), or experience in Internal Medicine, Cardiology, Nephrology or Case Management
• Spanish-speaking preferred in some markets

Responsibilities

• Manage complex patients, focused on reducing hospitalizations and improving health outcomes
• Provide primary care directly in nephrologist offices and dialysis centers
• Help establish new practices and be comfortable being among the first hires in a new organizational structure
• Deliver care to patients with complex, chronic illnesses
• Work autonomously and in collaboration with all members of the healthcare team to coordinate and facilitate quality, consistent, cost-effective care
• Participate in innovation and care redesign
• Grow into future leadership roles
